LCSWs act as advocates and help clients gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse.

Chaplains are members of the team who specialize on providing guidance and support to patients and families. Chaplains may visit with families and provide resources or information on hospice.

Certified Nursing Assistants (CNAs) will assist patients daily with tasks including personal hygiene(showers, giving bedpans, shaving) and activities like feeding patients or positioning patients. Providing patient comfort is paramount. CNAs will need to complete forms, reports, logs, and records.

Individuals will work as a case manager for patients and families. They will need to complete ongoing assessments of the patient and family in order to better care for and understand the patient’s needs. Provides evaluation of patient’s status, and creates a care plan. The highest level of hospice care for patients and families is expected. Experience working in a hospice setting is ideal.

Builds market position by locating, developing, defining, negotiating, and closing business relationships.

The Community Liaison must have experience working in a marketing and/or business development capacity within the hospice/hospital/medical community. Sale/Marketing experience is preferred.

Additionally, the ideal individual will be personable and outgoing and will be responsible for opening doors throughout the territory to referral sources and other types of providers. This individual will also serve as an educational resource to the community and patient family members regarding hospice care.
